Chronic Skin Conditions

Causes and Management of Coin-Shaped Eczema

Nummular eczema is characterized by round red lesions and severe itching. To reduce recurrence, it is important to look at both the skin condition and lifestyle factors.

In the clinic, I often see patients who have round red marks like coins and lose sleep every night because of itching. At first, they may think it is a small patch of eczema, but scratching and repeated irritation can cause the area to spread. With nummular eczema, itching, sleep, and stress can influence one another as much as the visible lesions do, so it is important to set the right direction early.

What Does Nummular Eczema Look Like?

As the name suggests, nummular eczema is characterized by round or oval coin-shaped lesions. They may become red and raised, with small bumps or blister-like changes. It tends to be very itchy, and many people feel more uncomfortable in the evening or at night.

When scratching continues for a long time, the skin barrier can become weaker, leading to cracking, oozing, and crusting. If it recurs for a long time, the skin may thicken and leave darker discoloration, so it is best not to dismiss it as simple dryness.

There Is More Than One Reason It Recurs

The factors that affect nummular eczema differ from person to person. Some people worsen after prolonged exposure to a humid environment, while others notice more itching when they eat a lot of flour-based foods or instant foods. It is also common for the skin response to become more sensitive as overwork and stress accumulate.

That is why I do not look only at the shape of the lesions. I also look at constitution, the current stage of the skin condition, the living environment, sleep, and stress. From a Korean medicine perspective, I also check how heat metabolism and circulation within the body may be placing a burden on skin recovery.

Precautions When Using Ointments and Medications

When itching is severe, antihistamines or steroid ointments may be prescribed. These treatments may help calm inflammation and itching, but if the underlying lifestyle factors or body imbalance remain, discomfort can return.

If you already have prescribed medications or ointments, do not stop them or increase the amount on your own. We recommend discussing adjustments with your medical team. In particular, ointments that have been used for a long time are safer to manage gradually while monitoring the skin condition rather than stopping suddenly.

When Consultation Is Recommended

  • When coin-shaped red lesions keep recurring
  • When itching frequently wakes you from sleep or it is hard to stop scratching
  • When the skin cracks and is accompanied by oozing, crusting, or pain
  • When the lesion area thickens or leaves dark discoloration
  • When it often recurs despite using ointment and you want to review your management direction
  • If there is severe heat, rapid spreading, or fever, we recommend seeking care promptly

Nummular eczema requires not only calming the inflammation on the surface, but also creating conditions that allow the skin to recover. Checking diet, sleep, stress, and skin irritation one by one and making a treatment plan that fits your body can help reduce unnecessary aggravation. Rather than enduring it for a long time, we recommend checking the condition early.
