In the consultation room, I often meet patients who come in because round eczema on the arms and legs does not improve easily. In many cases, it begins as small red spots, then becomes itchy and scratched, forms scabs, oozes again, and spreads over a wider area. Nummular eczema is a form of chronic eczema that appears coin-shaped, as the name suggests, and it tends to recur too often to be viewed simply as a problem caused only by dry skin.
Features of Coin-Shaped Eczema
Nummular eczema may appear with small papules or blister-like lesions on top of red, swollen erythema. Itching may worsen at night and disturb sleep, and unconscious scratching can lead to cracked skin or oozing. If this condition continues, the skin barrier can become weaker, creating a cycle in which the skin reacts sensitively to external irritants.
What to Consider When It Keeps Returning Despite Ointment
Antihistamines or steroid ointments may help calm itching and inflammation. However, if symptoms recur, we need to look not only at the area where the medication is applied, but also at why the skin barrier breaks down so easily and why inflammatory responses in the body persist. If you are already using prescribed medication, it is safer not to stop it on your own and to discuss the duration and method of use with your medical team.
What We Look at in Korean Medicine Care
When I see nummular eczema, I do not look only at the shape of the lesions. I also check constitution, digestion, sleep, stress, heat sensations, sweating, and circulation in the hands and feet. The treatment direction may differ depending on whether heat is gathering in the skin, circulation is blocked and recovery is slow, or dryness and oozing are present together. We combine herbal medicine, acupuncture, moxibustion, pharmacopuncture, and, when needed, bloodletting therapy according to the stage of symptoms, with the goal of supporting skin barrier recovery and easing inflammation.
When We Recommend a Consultation
- When round eczema lasts more than 2 weeks or gradually grows larger
- When itching interferes with sleep
- When cracking, oozing, and scabbing keep recurring
- When it returns in a similar area after stopping ointment
- When redness spreads quickly or is accompanied by heat, pain, or pus
At home, it is important to reduce hot showers and harsh cleansers, and to apply enough moisturizer right after washing. Choose loose cotton clothing, and when it itches, cooling and calming the area rather than scratching may help reduce skin damage. Nummular eczema requires a process of supporting the body’s recovery conditions along with addressing the visible lesions. If recurring eczema is making daily life uncomfortable, we recommend checking your current skin condition and constitution together.
