Chronic Skin Conditions

What to check when seborrheic dermatitis keeps recurring

If redness, itching, and flaking of the face and scalp keep recurring, we need to look not only at the surface of the skin but also at sebum, inflammation, heat sensations, and daily rhythm.

In the clinic, I often hear patients say their face keeps getting red, flakes form around the nose and forehead, and even the scalp is itchy. It often seems to improve, then flares up again after fatigue or eating irritating foods, which can be emotionally draining. Seborrheic dermatitis is often not just a problem of visible skin symptoms, but a condition in which sebum production, inflammatory response, stress and sleep, and the body’s heat balance are interrelated.

It commonly occurs in oily areas

On the face, seborrheic dermatitis is common in areas with higher sebum production, such as the sides of the nose, between the eyebrows, the forehead, and around the chin, and it can also appear on the scalp and around the ears. Redness, itching, fine flaking, and stinging can recur, rising and falling depending on your condition. Some people feel it starts like dandruff on the scalp and spreads to the face.

It is difficult to judge by flushing alone

Not every red face is the same condition. It can appear to overlap with rosacea, contact dermatitis, acne, or allergic reactions, so we need to check the location of the lesions, the pattern of flaking, itching, and the history of cosmetics and ointments you are using. In seborrheic dermatitis, oily flakes or small bumpy inflammation often accompany a red base.

In Korean medicine, we look at heat and circulation together

When I see seborrheic dermatitis, I look not only at inflammation on the surface of the skin but also at patterns of heat concentrating in the upper body, digestive status, sleep quality, and accumulated fatigue. When the body’s recovery capacity is reduced, inflammation can last longer even after minor irritation, and heat sensations in the face and scalp may not settle easily. Korean medicine treatment is approached in a way that regulates excessive heat and supports circulation and skin recovery according to the individual’s constitution and symptoms.

Use ointments carefully

If you are using steroid ointments, antifungal agents, or similar treatments, it is better to review the duration and method of use through medical care rather than stopping them suddenly on your own. They can be necessary treatments when symptoms are severe, but some people feel their skin has become more sensitive when they use them repeatedly for a long time without addressing underlying management. If you are currently using dermatology medications, bringing the names of both oral and topical medications can help us make a treatment plan.

Consultation is recommended in these cases

  • When facial redness, flaking, and itching recur for more than 2 weeks
  • When scalp dandruff and itching worsen together with facial symptoms
  • When an ointment calms symptoms briefly but they often recur
  • When symptoms clearly worsen after spicy food, alcohol, or lack of sleep
  • When oozing, pain, or sudden swelling is present, or infection is suspected

Lifestyle management is also part of treatment. It is best to cleanse briefly with a low-irritation product and avoid forcibly rubbing off flakes. Alcohol, greasy foods, and excessive spiciness can increase heat sensations and itching, so we recommend reducing them when symptoms are flaring. Adequate sleep and light aerobic exercise can help restore the skin’s recovery rhythm.

Because seborrheic dermatitis can easily repeat cycles of improvement and worsening, it is important to look at why it recurs rather than only covering the immediate redness. Checking skin condition and the body’s signals together, while continuing manageable treatment and lifestyle adjustments, may help reduce the degree of symptom fluctuation.
