In the clinic, I often meet people who are worried about eczema that spreads in a round shape. At first it may look like small red spots or blisters, but as scratching and oozing repeat, it can grow into a lesion with coin-like borders. When it appears on visible areas, it can make you feel self-conscious, and itching may worsen every night and disrupt sleep.
What does nummular eczema look like?
Nummular eczema is dermatitis characterized by round, coin-shaped eczematous lesions. Small vesicles and papules cluster together, becoming red and oozing, and over time scabs or scaling may also appear. It can occur on various areas, including the backs of the hands, tops of the feet, arms, legs, and face, and can affect people from infancy to adulthood.
Why does it tend to recur?
The cause is not fixed as a single factor. However, when the skin barrier weakens and its ability to hold moisture decreases, dryness and inflammatory reactions can overlap, making symptoms more likely to occur and last longer. Fatigue, lack of sleep, stress, frequent irritation, and use of products with strong cleansing power can also make the skin more sensitive.
In Korean medicine, we do not look only at inflammation on the surface of the skin; we also assess when itching worsens, the pattern of oozing, digestion and bowel movements, sweating, heat sensations, and fatigue. Even with the same nummular eczema, the treatment direction may differ for those with prominent dryness, those with strong heat and redness, and those with repeated oozing.
Treatment looks at both the skin and the body’s condition
The treatment goal is to reduce itching and inflammation and to create an environment in which the damaged skin barrier can recover. Herbal medicine tailored to constitution and symptoms is prescribed while considering chronic inflammation, skin dryness, reduced circulation, accumulated fatigue, and other factors together. Acupuncture, moxibustion, and Cupping treatments may be used together for the purpose of supporting qi and blood circulation, easing tension, and helping skin recovery.
Topical products should be selected according to the condition of the lesions. This is because the care needed during periods with a lot of oozing differs from the care needed when the skin is dry and cracked. If you have already been prescribed and are using steroids or immunomodulators, do not stop or change them on your own; it is safer to adjust them with the Medical Team while monitoring the skin condition.
Key points for lifestyle management
- Do not shower for a long time with hot water; wash briefly with lukewarm water.
- After showering, gently pat the skin dry and apply moisturizer right away.
- It is best to avoid rough fabrics, tight clothing, and strongly scented cleansers.
- When itchy, soothe the area with coolness rather than scratching, and keep nails short.
- Keep a steady daily rhythm so that lack of sleep and overwork do not repeat.
We recommend a consultation in these cases
- When coin-shaped lesions gradually grow larger or spread to multiple areas
- When oozing, yellow scabs, heat, or pain is present
- When itching causes frequent waking or makes daily life uncomfortable
- When pigmentation changes or skin thickening is seen
- When symptoms repeatedly improve and worsen
Nummular eczema is difficult to judge simply by how it looks on the surface, and if it is left untreated for a long time, the skin may become more sensitive. I believe it is important to look at the severity of symptoms, lifestyle habits, and overall condition together and create a treatment plan that is not excessive. If you are concerned about recurring eczema, I recommend checking your current skin condition.
